The Federal Motor Carrier Safety Administration is renewing a voluntary form for medical examiners. The form helps assess drivers with non-insulin-treated diabetes for commercial driving certification.

The Federal Motor Carrier Safety Administration is renewing an information collection request. The request covers the Non-Insulin-Treated Diabetes Mellitus Assessment Form, MCSA-5872.

The form is voluntary. Certified medical examiners may use it when issuing Medical Examiner Certificates to commercial drivers.

The form applies to drivers diagnosed with non-insulin-treated diabetes mellitus. Medical examiners can use it to contact the healthcare providers who manage the drivers diabetes care.

The information helps medical examiners decide if a driver meets the physical qualification standards for operating commercial motor vehicles.

The agency submitted the renewal to the Office of Management and Budget for review. The agency also invited public comment on the collection.
