A voluntary assessment form for medical examiners will be renewed. The form helps examiners check if drivers with non-insulin-treated diabetes meet federal safety standards. The agency is now asking for public comments on the renewal.
Medical examiners can use this form to get information from a driver's regular doctor. The information helps the examiner decide if the driver meets physical qualification standards. The form is not required. Examiners may choose to use it when they need more details about a driver's health management.
The agency plans to send the renewal request to the Office of Management and Budget for approval. The form is available as a fillable PDF. Doctors can send the completed form to the examiner by fax or email. About half of the forms are expected to be sent electronically.
Carriers and drivers who want to share feedback must submit comments by October 26, 2026. Submissions must include docket number FMCSA-2026-1783. Comments can be sent online, by mail, by hand delivery, or by fax. All submissions will be posted publicly without changes.
